13. How Do I Make A Claim And Receive My Payment?
If youd a PayPal account between October 1, 1999 and January 31, 2004, you can make a claim and receive a payment. If you do not make a claim, you will not receive a payment. To make a claim, complete and submit the online Claim Form contained on this website. The Claim Form must be completed by October 23, 2004. The Claim Form will require you to print out, sign, and mail in a certification and signature page. That page must be mailed by November 1, 2004 to:
